Summary:

Mountain View Los Altos High School District is requesting Change Order #3A with Gonsalves & Stronck be ratified:
- Facility Repairs Inc 2: The plans called out for 10” Skylights, however, the clips attached to the Skylights need blocking which was not shown on the plans. The design intent was to use curb cap, but 10” skylights do not come with curb cap. Contractor to Provide the material and labor for this added
blocking for the Skylights.
- 500 Wing Replacement: Provide all labor, equipment, and material to excavate & off haul for deepened foundations. Provide stepped footings and fill trenches with lean concrete/slurry after excavation as described in Request for Information #21.3 and Construction Change Directive #5 (CCD#5). Contractor shall also excavate around unforeseen existing pool pipes in conflict with the building
footing and provide a slurry encasement around the pipes per CCD #5.
- Facility Repairs Inc 2: Provide labor, equipment, and material to sawcut the concrete floor at classrooms 408, 409, and 410 as needed to accommodate in-slab electrical conduits to bypass doorways as described in Request For Information #11.1. Contractor shall also provide concrete patch back following completion of the electrical scope. Carpet and tile replacement will be completed under a separate work directive utilizing the contract allowance. |
